Job Title: Physical Therapist
Shift Timings: 06:00 PM to 03:00 AM PST
Employment Type: Full Time and Remote

Position Overview:
We are seeking a detail-oriented and compassionate Physical Therapist to join our team. The ideal candidate will be responsible for conducting assessments, creating rehabilitation plans, and supporting patients with chronic, acute, or complex physical conditions through effective care planning and patient communication.

Key Responsibilities:
Conduct assessments related to mobility, strength, posture, pain, and functional limitations.
Review patient profiles and document findings based on clinical criteria.
Communicate with patients via outbound/inbound calls or in-person sessions to explain care plans and answer queries.
Develop and maintain personalized rehabilitation plans, including prescribed exercises, goals, vitals, progress tracking, and documentation.
Coordinate appointment scheduling and collaborate with healthcare providers as needed.

Requirements:
Doctor of Physical Therapy (DPT) degree.
Prior experience in clinical or rehabilitation care planning roles is preferred.
Strong understanding of chronic condition management, post-surgical rehabilitation, and patient counseling.
Excellent English communication and analytical skills.
